Bright Eyes Dog Rescue Proxy Form for the Annual General Meeting, to be held at 2:00 PM on October 19, 2024

The above being a member of Bright Eyes Dog Rescue ("BEDR"), hereby appoint Heather McClinton, Chair, as my proxy to attend, act, and vote for me at the Annual General Meeting (the "Meeting") of the members of BEDR to be held virtually via Google Meets on October 19, 2024 commencing at 2:00 p.m., or any adjournment thereof, and at every poll which may take place in consequence thereof, to the same extent and with the same power as if I were personally present at the Meeting. A person acting as a proxy must be in attendance at the meeting. The Proxy Form is prescribed by the BOARD of BEDR pursuant to the provisions of Bylaw #11. This Proxy Form is solicited on behalf of the Board of BEDR. I can over-ride the above Proxy holder by striking out the above and assigning a chosen Proxy in the entry below.

This Proxy Form must be signed and dated by the member or his or her attorney authorized in writing. To be effective for the Meeting, properly executed Proxy Forms must be received by the Board of BEDR via this form before 5:00 p.m. on October 18th, 2024. After such time, the Chairperson of the Meeting may accept or reject a Proxy Form delivered to him or her in his or her discretion but is under no obligation to accept or reject any particular late Proxy Form.
